🦠 Herpes: Understanding a Common Viral Infection

Herpes is one of the most common viral infections worldwide. While there is currently no cure, effective treatments can help manage symptoms, reduce outbreaks, and lower the risk of transmission, allowing many people to live healthy, active lives.

HISTORY / OVERVIEW

Herpes simplex is caused by the herpes simplex virus (HSV), which remains in the body after the initial infection and can reactivate periodically.

There are two main types:

  • HSV-1: Most commonly associated with oral herpes (cold sores), but it can also cause genital herpes.

  • HSV-2: Primarily associated with genital herpes, although either virus type can infect the oral or genital area.

Many infected individuals have mild symptoms or no symptoms at all, yet they can still transmit the virus.

HOW HERPES SPREADS

Herpes can spread through:

  • Direct skin-to-skin contact with an infected area

  • Oral, vaginal, or anal sexual contact

  • Kissing (especially with active oral sores)

  • Contact with saliva or genital secretions

  • Transmission from a pregnant person to the baby during childbirth in certain situations

The virus can sometimes spread even when no visible sores are present (asymptomatic viral shedding).

COMMON SYMPTOMS

Some people experience no symptoms, while others may develop:

  • Painful blisters or sores around the mouth or genitals

  • Tingling, itching, or burning before sores appear

  • Pain during urination (with genital herpes)

  • Swollen lymph nodes

  • Fever and body aches during the first outbreak

  • Recurrent outbreaks that are often milder than the initial episode

DIAGNOSIS

Healthcare providers may diagnose herpes using:

  • Physical examination

  • Swab testing (such as PCR) from an active sore

  • Laboratory testing of lesion samples

  • Blood tests to detect HSV antibodies in selected situations

TREATMENT

Although herpes cannot currently be cured, antiviral medications can help:

  • Shorten the duration of outbreaks

  • Reduce symptom severity

  • Decrease the frequency of recurrent outbreaks

  • Lower (but not eliminate) the risk of transmitting the virus to sexual partners

Treatment options may include antiviral medications prescribed by a healthcare professional, such as Acyclovir, Valacyclovir, or Famciclovir.

PREVENTION

To reduce the risk of transmission:

  • Avoid sexual contact during active outbreaks.

  • Use condoms or dental dams consistently, recognizing they reduce but do not eliminate risk.

  • Consider suppressive antiviral therapy if recommended by a healthcare provider.

  • Inform sexual partners about your HSV status.

  • Avoid kissing or sharing items that contact saliva during active oral herpes outbreaks.
